Issuance of Quarry Purchase Slip/Stub (Quarry Receipts and Monitoring tickets)

About the Service:

Quarry Purchase stub/slip is imposed on all quarry permit holders or quarry operators before purchasing Official Receipts and Monitoring tickets used during the hauling and disposal of quarry materials..

Requirements:

a) Quarry Permit (for new applicants)

b) Authorization letter (if not the permitee)

c) Production Report (for existing operation)

Steps to Avail of the Service:

Issuance of Hauling/Loading Pass

About the Service:

All trucks/haulers/equipments intending to engage in quarrying activities within the territorial jurisdiction of the Province are required to secure a Hauling pass/permit pursuant to Tax Ordinance No. 

Requirements:

a) OR and CR of trucks or vehicle being 

b) Affidavit/proof of actual ownership

c) Pictures of trucks or vehicle (front, back and side)

Get schedule of Inspection of Truck/s
Check receipt and advise client on schedule of inspection of Truck/s;

Receive inspection report;
